Primary Care at CCCHC
Our Primary Care teams provide preventative care, chronic disease management, acute illness treatment, and ongoing wellness support for patients of all ages. As a Federally Qualified Health Center, CCCHC offers primary care to everyone — regardless of ability to pay.
Primary care is the foundation of a healthy life. At CCCHC, our board-certified family medicine and internal medicine physicians build long-term relationships with patients to provide consistent, culturally sensitive care across all life stages. We offer same-week appointments at most locations and accept Medi-Cal, Medicare, and most major insurance plans.
