The Pediatric Care CHDP Program at the CCCHC Hollywood Location
At the CCCHC Pediatric Program in Hollywood California, Child Health and Disability Prevention, or CHDP, is a program aimed at delivering health services and assessments to low-income children and youth residing in the state of California. Families are provided with care coordination, which can be broken down into assistance with scheduling medical appointments, getting the right treatment and diagnostic services, and transportation. The assessments are done by a range of involved professionals and providers, including local health departments, enrolled private physicians, and even a few local school districts.

Who Is Eligible?
Low Income Families – From birth to 19 Years, or Medi-Cal Participants up to 21 years. If your child has Medi-Cal, ask your doctor or health plan for CHDP check-ups.
