The Pediatric Care CHDP Program at the CCCHC Altadena Location
CCCHC has its own Pediatric CHDP Program based at our Altadena clinic. Here, we offer comprehensive health assessments in order to detect and prevent health problems which could affect youth and children in the low- income category. At this Pediatric Care clinic, eligibility for CHDP services extends to children in the low-income category that are under 19 years of age. If a child at or under 21 years is on Medi-Cal, he or she would be eligible to the CHDP as well. Children and youth can receive benefits whether or not they have an existing health problem. Referrals are made when a problem is found, and families are assisted in obtaining services regarding said problem or problems.
Our comprehensive array of primary care services include:
- Health history
- Physical examination
- Developmental assessment
- Nutritional assessment
- Dental assessment
- Vision and hearing tests
- Tuberculin test
- Laboratory tests
- Health education/anticipatory guidance
- Referral for any needed diagnosis and treatment.

Who Is Eligible?
Low Income Families – From birth to 19 Years, or Medi-Cal Participants up to 21 years. If your child has Medi-Cal, ask your doctor or health plan for CHDP check-ups.
