Navigating Cancer Care: A Comprehensive Guide to Support Services at CCCHC

Receiving a cancer diagnosis can be overwhelming, and navigating the complexities of cancer care can
feel daunting. At CCCHC, we understand the challenges that patients and their families face when
dealing with cancer, which is why we offer comprehensive support services to guide you through every
step of your cancer journey. In this blog post, we’ll provide a comprehensive guide to the support
services available at CCCHC, designed to empower and assist you in your cancer care experience.

1. Patient Navigation Services:

Our patient navigation services are designed to provide personalized support and guidance throughout
your cancer care journey. Our dedicated patient navigators work closely with you to:

 Schedule appointments and coordinate care with various specialists.

 Provide information about your diagnosis, treatment options, and supportive services.

 Connect you with resources for financial assistance, transportation, and lodging.

 Advocate for your needs and preferences within the healthcare system.

2. Psychosocial Support:

Coping with a cancer diagnosis can take a toll on your emotional well-being, which is why we offer
comprehensive psychosocial support services, including:

 Individual counseling: Our licensed counselors provide emotional support, coping strategies, and
therapeutic interventions to help you navigate the emotional challenges of cancer.

 Support groups: Joining a support group can provide a sense of community, shared experiences,
and practical advice from others who understand what you’re going through.

 Art therapy, meditation, and relaxation techniques: These complementary therapies can help
reduce stress, promote relaxation, and enhance overall well-being during cancer treatment.

3. Nutritional Counseling:

Proper nutrition plays a crucial role in cancer treatment and recovery. Our registered dietitians offer
personalized nutritional counseling to:

 Address dietary concerns and nutritional needs specific to your cancer diagnosis and treatment
plan.

 Provide guidance on maintaining a healthy diet during treatment, and managing side effects such as
nausea and loss of appetite, and optimizing nutrition to support healing and recovery.

4. Survivorship Programs:

Our survivorship programs are designed to support you beyond the completion of active cancer
treatment. These programs focus on:

 Survivorship care planning: Our healthcare team works with you to develop a personalized
survivorship care plan that outlines follow-up care, monitoring, and strategies for maintaining
overall health and wellness after cancer treatment.

 Survivorship workshops and events: Join us for educational workshops, wellness events, and
survivorship celebrations designed to support and empower cancer survivors on their journey to
recovery.

At CCCHC, we are committed to providing comprehensive support services to guide and empower you
through every step of your cancer care journey. From patient navigation and psychosocial support to
nutritional counseling and survivorship programs, we’re here to help you navigate the challenges of
cancer with compassion, expertise, and personalized care. You don’t have to face cancer alone—let us
be your partner in care at CCCHC.
